She is 90 years old and suffers from chronic obstructive pulmonary disease and adult-onset diabetes mellitus. Her sons are all adults and speak English fluently, and the family retains their Turkish culture and tradition of patriarchy. The family has requested only comfort care, and thus the cardiology consultant has concluded that Jamilah will not receive cardiac catheterization or be considered for bypass surgery. Jamilahs social worker has raised concerns, stating that in private the patient had asked for help, and that she wanted to live. The social worker believes that the relationship between the mother and her sons seem unsupportive. Without intervention, Jamilah will die within days. The care team has either to honor the patients wishes or to disregard them in favor of her familys direction.
