J. E. 14 Yr. Old F S: CC: Headache HPI: Patient is a 14 year old female who was brought in to the clinic by her mom for complain of headache, with onset of about 1-2 weeks. Patient reports gettin... g headaches that sometimes last 1-2 days. Pt. reports headache symptoms as pulsing. She rates her pain “3” out of 5 and not relieved with Ibuprofen/Naprosyn. She reports associated symptoms as: occasional upset stomach. She denies any aura or trigger. Julia reports that sleep helps, as well as Mine Craft game helps distract her. TV hurts her eyes. She says that her mom thinks so much time on computer and looking at her phone is causing her headache. PMHx: overall health as good. Childhood/previous illnesses: None. Chronic illnesses: None. Surgeries: None. Hospitalizations: None. Immunizations: UTD; receives flu shot annually. Allergies: Seasonal. Blood transfusions: None. Current medications: None. LMP: One week ago.
