He had tried several over-the-counter remedies and ultimately decided to seek professional medical attention when the symptoms were not improving. Upon physical examination, Dr. Peter Lee, the president and minority shareholder of APC, determined that Mr. White was a classic presentation of food poisoning and would probably improve with IV fluids and a two-week course of oral antibiotics. As he was completing Mr. White’s medical records, Dr. Lee noticed an alert from his intake nurse, which stated that as a government employee (he was a police officer), Mr. White had premium insurance coverage, which would cover just about any procedure – with no out-of-pocket cost to the patient – at excellent reimbursement rates. Dr. Lee was faced with a dilemma. On one hand, with over 25 years of medical experience as a doctor, he knew food poisoning was likely the correct diagnosis. On the other hand, the $450,000 computerized tomography (CT) machine the company had purchased the previous year was barely being used. Mr. White probably didn’t need a CT, but – mild radiation exposure aside – it wouldn’t necessarily hurt him either. Dr. Lee could even justify ordering the CT to Mr. White and to his insurance carrier: what if he was wrong and it wasn’t food poisoning, but something more sinister, like cancer?
