Chief Complaints (CC): chest pain and shortness of breath.
HPI: Mr. Hendrix is a 66-years-old Caucasian male who present to the health unit with complains of constant chest pain and shortness of breath for the last 3 days. He reports that the pain gets worst when taking a full breath. He described chest pain as “really sharp” and stated that “his heart feels like his heart is racing”. Patient verbalized nothing seems to be easing the pain and denies taking any medication. Patient reports hemoptysis in the morning and right calf redness with inflammation. He reports he was on a plane where he sat for 8 hours.
Medications: patient denies taking any medication
Allergies: NKDA
Past Medical History: No significant past medical history.
Immunization: Up to date on all his immunization including his annual flu vaccines. Last tetanus shot on 2015
Social History: client work as an insurance agent and make about $100,000 annually. He is married with two sons and enjoy going on summer vacations with his wife. He denies alcohol illicit drug use. Enjoy spending time with his grandchildren and taking them out for ice cream.
Family History: reports that his father was a longtime smoker and died of lung cancer. Mother is diagnosed with hypertension and CAD in 2001 and she still alive. His younger brother who is 63 years old diagnosed with hypertension 2010.
