Other countries have been applying public health concepts to chronic disease management, all while controlling costs. Unfortunately, the higher spending in the U.S. has not produced improved outcomes. The current volume-based model does not support population health. Population health management aims to reduce healthcare costs while managing health populations to improve patient outcomes. To support the transition to value-based care, the technology infrastructure must support the data reporting of patient results, social determinants of health, and procedure-based costs [ CITATION Pop21 \l 1033 ]. Transitioning the electronic health record system (EHRS) at Paducah Health Medical System (PHMS) is necessary to integrate data analytics to promote efficiency, improve overall quality patient care and health outcomes, and support community public health.
