Without adequate intervention for meeting the population's health needs, chronic diseases' prevalence will continue to rise and continue to create a financial burden on the nation. Evidence has shown meeting people in their community where they live, and work encourages participation in their disease management. Exploring new innovative models will reduce the health disparities seen among the vulnerable populations and people suffering from mental diseases. Mobile Health Clinic (MHC) is an innovative healthcare delivery model that provides care to residents in the community.
