Both of which can and will happen from time to time. Human errors happen when we, the coder, miss reads the patients diagnosis and the chart before fully understanding his procedure or illness and rush to get the coding done. And systematic errors happen when the coder does not understand what they are reading in medical terms or lack the knowledge to understand the coding rules and changes. When reading the medical record, it is important to read it thoroughly. TheÂ coder has to look for the diagnosis and the procedure that was performed and be able to understand and interpret what the coders are reading. But remember the rule, there are always rules to go by. Some codes require five digits, and some need seven digits, and then some modifiers mightÂ need to be put in place.
