Â Describe the sentinel event. On Thursday, September 14, 2018 at 12:30 pm, a sentinel event occurred at the Nightingale Community Hospital Ambulatory Surgical Center in which a minor surgical patient was incorrectly discharged to a non-custodial parent. The patient, a three year-old female, initially presented to the hospital for a scheduled outpatient surgery in the care of her mother. The patient was registered by Katie Jessup, Registrar, according to policy. She was then escorted to the pre-operative area by Greta Doppke, Pre-Op Nurse, still in the company of the mother. Doppke prepped the patient and advised the mother that the procedure would take approximately 45 minutes to complete, followed by approximately one hour in recovery. The mother advised Doppke that she needed to leave the hospital during the procedure but would return within the estimated procedure/recovery time to pick the child up. The mother provided Doppke with her mobile number and requested that she be called if the procedure completed ahead of the estimate provided by staff. Doppke stated that she recorded the number on her note pad. The patient was then picked up by Rosemary Frye, O.R. Nurse.
