Access to care, transportation, income, insurance, where a person was born, where they live, and/or their support system are just a few examples of factors that affect a persons state of health. “Social determinants of health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks”(U.S. Department of Health and Human Services, 2018, para. 4). Determining a persons social determinants happens during the interview process of a patients assessment. Though social determinants can be considered a sensitive subject by some, gathering the information is imperative to developing the best healthcare plan for that specific patient.  The strategies used to identify and assess the patients social determinants were interviewing skills conducive to developing a rapport with the patient. Building a relationship with the patient, so that they feel they can be open and honest, is important to the accuracy and completeness of the answers provided to the questions being asked.
