The interview happened after an emergency during a staff member's lunch break. The staff member noted that a patient's systolic blood pressure dropped over 30 points. This was documented in the patient's electronic chart, with no intervention noted. The staff member did not give other staff a handoff report but left the clinic area for a lunch break. During the staff member's lunch break, the patient became unresponsive, and the blood pressure dropped a substantial amount. Had a handoff report been utilized, other staff, including the charge nurse, would have been aware of the need to monitor the patient closely. The interview consisted of reviewing what happened in this specific situation and taking notes. There were also chart audits performed to find out if other similar events had happened previously.
