Patients labs are drawn weekly, and medication dose adjustments are completed per algorithms from each new lab result. If a nurse administers medications without checking for updated dose changes before administration, the incorrect dose may be administered, resulting in a medication error. These medications are titrated very closely, and not verifying the new dose changes can lead to critical medication errors and cause harm to the patient. For example, a charge nurse administered medication without checking the electronic algorithm and most current labs. By this, she administered a medication that, per the algorithm, should have been held. Although the patient did not suffer injury, this will negatively impact the quality outcomes for the clinic.
