This paper is going to discuss a little about each of the following documents, consent of information, release of information, treatment plan, including treatment goals, clinical or SOAP notes, and discharge plan. The purpose of this paper is to identify the documents, the purpose of the documents, explanation of why the documents are in a clients chart, and ethical standards, legal requirements, and professional standards of having accurate record keeping of each of the documents. Consent for Treatment Consent for treatment is a document provided by the counselor that authorizes that the individual is agreeing to receive any form of treatment. This form is an informational form that explain the clients rights and what they should expect from treatment. The purpose of this document is to make sure the client is informed of all of the services that could be provided during treatment and the client is prepared for the benefits of treatment (OHara, 2020).
