Energy metabolism of the heart is to provide an adequate supply of high-energy phosphate compounds to replace the continuous use of ATP in contraction, in ionic exchange processes, and also for othe... r energy-demanding processes. Due to very high turnover rate of ATP in the myocardium, a correspondingly high rate of mitochondrial production of ATP is required. Myocardial function depends on ATP supplied by oxidation of several substrates. In the adult heart, this energy is obtained primarily from fatty acid oxidation through oxidative phosphorylation.2 However, the heart energy source may change depending on several factors such as substrate availability, energy demands, oxygen supply, and metabolic condition of the individual. In the post-absorptive state several hours after a meal, the heart utilizes fatty acids (70%) and carbohydrates (30%). Following a high carbohydrate meal, the heart can adapt itself to utilize carbohydrates (primarily glucose) almost exclusively. It cans also use lactate especially during exercise.
