An old female of Korean descent came to her first appointment following 21 days of hospitalization for acute ma... nia onset. The patient is alert, oriented with impaired insight. The patient denies any suicidal and homicidal ideation. The patient denies any delusional and paranoid thinking. The patient mood is happy and euthymic. Patient judgment is intact. Speech is rapid, pressured, tangential. Affect Board. The patient is dressed in an evening gown for her appointment. The patient is hyperactive and energetic. The patient has noted with flights of the idea. The patient slept 5 hours last night. The patient is physically healthy, and lab studies are within the normal range. Genetic testing comes back positive for the CYP2D6*10 allele. The patient is non-compliant with medication. The patient stopped taking her lithium since she was discharged from the inpatient hospital. The young mania rating scale score is 22. Introduction Bipolar disorder is a recurrent and chronic disorder characterized by fluctuation in mood and energy(Bonnin et al., 2019). People with bipolar frequently experience persistent residual symptoms. The patient with bipolar disorder has problems in psychosocial functioning, cognitive impairment, and poor quality of life(Bonnin et al., 2019). Patient with bipolar disorder exhibit symptoms of mania with extremely elevated mood, energy, unusual thought pattern and sometimes psychosis and depression(Martinowich et al., 2009).Patient in given scenario is positive with CYP2D6*10 allele. The CYP2D6 allele plays a vital role in the metabolism if 20 to 30 % of therapeutic important drugs mainly including antipsychotics, antiarrhythmic, tricyclic antidepressants, beta blockers and opioids(Zheng et al., 2017). The genetic polymorphisms in CYP2D6 cause the tremendous variation in CYP2D6 enzymes activities(Zheng et al., 2017). The purpose of paper is to explain the three treatment decision points, factors that might impact the 3 pharmacokinetics and pharmacodynamics processes and explanation of specific patient factors that may impact decisions(Zheng et al., 2017).
