Patient, Erica James, 29 year old African American female, presents in the
office for a physical. Purpose of physical is a general checkup before her
30th birthday.
On assessment, patient is 172 lbs.; height is 5 foot 3 inches; vision is
20/25; BMI-30.5.
Vital signs are documented as: BP-101/68; Temperature- 97.7 °’; Heart
Rate- 67/min; Respiration-17/min; SaO2- 100%; Pain score-0.
Patient has no known allergies, nonsmoker, non-drinker. Patient stated that
she does not currently exercise and mainly sits at her current job as a
secretary. Patient eats mainly Caribbean foods, fast foods and skips
breakfast. Patient’s family history includes breast cancer. Patient stated she
is ready to take the necessary steps in order to be healthier, not only for
herself but also her child. The patient is very attentive and listening keenly
to the nurse. She takes out her iPad and begins taking notes and asking
questions about diets she has researched online. The nurse asks questions
about the patient’s previous attempts at losing weight and patient stated
that she tried in the past but wasn’t serious, but now is motivated after
seeing a close friend become ill. Patient appears to be tech savvy, as the
patient shows the nurse a spreadsheet she began of the foods she currently
consumes daily. There appears to not be any knowledge gaps with patient
and patient stated they are very comfortable navigating different types of
technology, given her job as a secretary.
Advised patient on the Weight Watchers mHealth application as an extra
tool to help reach her goals and patient was eager to get started. Patient

1. Information about the mHealth application:
The purpose of the Weight Watchers app is “to get access to food and
fitness trackers, thousands of delicious recipes, and the support you need to
lose weight and build healthy habits for life” (Apple App Store, 2019). The
mHealth app will be able to help users keep track of their food and water
intake, create recipes which can coincide with their diet without having to
compromise foods that they love to eat. Users are able to scan the barcodes
of foods in the supermarket to see if it aligns with their diet and gives
education on what foods the person may need more or less of in their diet.
There are also member support groups embedded in the app to motivate
each other on their own weight loss journey. Support is available 24/7 to
members.
