Collaberation_with_the_Community.doc Clinical Field Experience B: Collaboration with the Community Grand Canyon University: SPD 521 – Collaborations and Communications in Special Education The small community of Scappoose, population of approximately 7,000, has resources from the county that help with “comprehensive case management and service coordination, our staff assists with early intervention and phase of life transitions.†(CCMH, 2019) The Community Developmental Disabilities Program within Columbia County Mental Health (CCMH) “provides access to local, regional, and statewide resources for children and adults who experience a developmental disability. Our team will work closely with eligible individuals to ensure needs are addressed in a comprehensive and collaborative manner.†(CCMH, 2019) CCMH is instrumental in organizing and stabilizing the wraparound service program for students and families that need that level of support. The local school district, Scappoose School District (SSD) is also a source of resources and supports for individuals with disabilities. SSD is the primary entity that oversees support for individuals with disabilities. This is due to the small size of the community. According to Tami McDonald, School Counselor, “For more serious or significantly impacted students they may also have anywhere from mild to significant involvement with CCMH DD offices.â€Â The North West Regional ESD also provides some outreach services to students per their IEP's.
