Most people that are seen at these clinics are typically there for routine health visits and wellness visits as these clinics are designed to diagnose and treat patients. These clinics can be a physicians private practice or be part of a large health care organization. The population who would use primary care clinics would families and would range from birth to geriatrics ("The Case For Primary Care", n.d.). Some key characteristics of primary care were found to be decision support for evidence based medicine: the care teams would make sure that patients get evidence-based care and treatment, making guideline-based reminders in the electronic health record (EHR) to other providers, having the clinic manager run regular reports to show gaps of care, and avoid test orders that are not related to change management. Risk stratified care management: making sure that each patient receives proper treatments based on their needs. Careful selections of specialists: providers typically have a list of specialty providers that they trust to refer their patients to.
