C229 Problem Description C229 Problem Description For this discussion, I had several topics to choose from. Yavapai county is very rural and therefore has a large population who have difficulty finding primary and specialty care. We also have very high obesity and homelessness rates. However, I decided to focus on the rate of under-immunized children and adults in Yavapai county. Community Health Nursing Diagnosis Statement Risk for infection, illness and disease in Yavapai County as evidenced by low levels of childhood vaccination and immunosuppression. Primary Prevention Topic Herd immunity is defined by Merriam-Webster as “a reduction in the probability of infection that is held to apply to susceptible members of a community in which a significant proportion of the individuals are immune because the chance of coming in contact with an infected individual is less†(“Herd Immunity Medical Definitionâ€, n.d.). What that means is that the members of a community who are more at risk to contract an illness are less likely to actually contract that disease because with more of the community immunized, there are less infected people to pass the disease along (Ventola, 2016). Those in the community who are more at risk to contract a disease can be immunocompromised because of chemotherapy treatment (to treat cancer) or because of any number of disorders that prevent them from receiving immunizations.
