The organizational problem under study is the patient education program™s impact on improving patient™s health literacy. Health literacy has been defined as "the degree to which individuals have the capacity to obtain, process, and understand basic health information and services needed to make appropriate health decisions (Nutbeam, 1998; Institute of Medicine 2004; Elgin, 2018). Due to the federal nature of the MSC, certain decisions are made at levels far removed from those with operational knowledge of the issue; thus this fact introduces factors that can present challenges for change within the organization. Internal leaders are restricted with particular types of decision making. The initiatives with the most transformative effect on the organization are often reserved for national leadership approval. One example of this was in the early 1990s, when the organization found it difficult for contracting and pay reasons to recruit psychologists, psychiatrists, pharmacists, radiologists, nurses, and other professionals, the NCVAMC agreed to hire 75 staff using its Title 38 authority to work at the hospital, although that arrangement was subsequently determined to be illegal by the Navy's judge advocate general (Harnly, 2005).
