The organization should develop a Health Information Management committee of patient care team members that will consider the needs for immediate and long-term patient care and make the decision on what is considered clinical meaningful information that is used when defining the legal, organization and professional areas of the LHR. The committee should first consider the legal issues by identifying the relevant regulations, standards and laws in defining the legal health records. The entities that enforce these standards should be identified and their definitions of the LHR should become the basis for the organizations own definition. Next the committee should consider organizational issues by determining if the records are created in the ordinary course of business. Reports, findings and treatment records with clinical decisions should be considered part of the LHR.
