week8assign 6050 .doc NUR 6050  ADVOCATING FOR THE NURSING ROLE IN PROGRAM DESIGN AND IMPLEMENTATION NURSE-DRIVEN DISCHARGE PLANNING PROGRAM CLASS: NUR 6050  The purpose o... f this interview is to introduce a Nurse-driven discharge planning program, to describe the benefits and the impact that effective discharge has in the community and to elaborate on the nurse's advocacy role in discharge planning. This program is geared towards implementing a standard and effective discharge planning process that can be used in Baltimore city area hospitals. The program will act as a bridge to help hospitals develop effective partnership with the key stake holders who are involved in discharge planning.   Interviewer: Good morning Nurse Kamau, Welcome to the program. Nurse Kamau: Good morning, thank you for having me. Interviewer: Tell us about your program. Nurse Kamau: Nurse-driven discharge planning program, is a program designed to improve acute care discharge experience, promote timely discharge and avoid premature hospital readmissions. Poor discharge planning in high risk populations leads to unnecessary readmission and emergency room visits. We hope the establishment of this program will eliminate those occurrences. Interviewer: What are the projected outcomes? Nurse Kamau: The main goal of the program is to improve patient outcome by improving the transition process.
