Health promotion and prevention begins in our homes, schools, workplaces, neighborhoods, and communities. Most people know eating right, getting enough sleep, exercise, not smoking or quitting smoking, regular physicals and vaccinations all influence our health. It™s the medical team™s job to make sure we not only treat but promote health and healthy habits to our patients and community to have a healthier population. After interviewing the McPherson family, I was able to focus in on areas they can work on to becoming a healthier family for years to come. Social Determinates of Health Social Determinates of Health (SDOH) are the œconditions of living, such as housing, socioeconomics, transportation needs, quality of education, that directly affect health and access to health care needs (Faulkner, 2018; Social Determinants of Health, 2014). This is important to consider for patients that want to be health but may not have the resources to achieve their health goals. Proper SDOH screening could improve the health care teams' ability to understand the ways that can impact patient health and ability to care for themselves, make informed clinical decisions and clinicians to make the right community referrals for their needs (LaForge et al.,2018).
