Findings Analysis Patient K.C. is a 25 year-old Hispanic female who presents to establish care with a new provider. Patient denies any current illness, complaints, or pain at this time. Patient does not currently take any prescription or over the counter medications or supplements. Patient denies any medication allergies. Childhood, adult, Ob/Gyn, and psychiatric medical history is negative. Surgical history is positive for a mid-back cyst removal and wisdom teeth extractions. Patient is up to date on most vaccines, but denies receiving an annual flu vaccine, and has never received a pneumonia vaccine. Familial history is negative for any significant diseases or illnesses. Patient is an occasional drinker, denies use of recreational drugs, and is a non-smoker. Patient reports a semi-healthy diet and good appetite, but does not exercise regularly. Social determinants are as follows: patient was born in Mexico, has a Bachelors degree, works a part-time job as a hotel front desk clerk, is married without any children, has reliable transportation, has insurance, lives in a safe neighborhood, is fluent in both English and Spanish, has access to technology, and has several interest and hobbies including animal rescue.
