According to the World Health Organization, adverse events resulting from medication errors are considered one of the most frequently encountered patient safety issues in clinical settings. As a nurse in an outpatient oncology clinic, one of my responsibilities is to administer chemotherapy to patients. The dose, rate of infusion, length of infusion, and the volume of the medication to be infused are calculated for us and programmed into the infusion pumps, and inputted into the EHR. Here is an example, we had a patient whose weight was entered as 161lbs in their electronic health record, and the dose of the chemotherapy was calculated based on that weight. As I was reviewing the patient's vitals, lab results, and previous assessments, I noticed that there was an increase in her weight since the last time she was in clinic two weeks prior. After assessing and asking the patient about the weight gain, I found out the weight should have been entered as 116lbs instead of 161lbs. If this error were not captured, the patient would have received a higher dosage of the chemotherapy than her body could have handled and caused a possible adverse event.
